Dentist or Home Teeth Whitneing?


Once you have decided to include teeth whitening as part of your affordable monthly / annual beauty regime the next big question is whether to visit your own dentist or to buy a teeth whitening kit that you can do yourself at home from a store / online website.

Dental Whitening
The benefits of visiting your dentist is that they will make near perfect 'mouth moulds / trays or impressions of your teeth, however this part of the process is time consuming and therefore expensive. The actual material the dentist uses to make the impressions with is very cheap; yet you can be charged up to £100 for an upper and lower set, for just this part of their process alone. So Why Are Your Teeth Not White


We all want white teeth and for a variety of reasons. In today's modern society we tend to spend an awful lot of money on our 'beauty' up keep and now teeth whitening has become a trendy & affordable part of our beauty regimes. But why are our teeth NOT white?

There are several reasons why we all don't have the same gleaming white smile. however we will focus on the two main types, extrinsic and intrinsic staining.

Responsible for the extrinsic staining are the usual suspects, tartar, red wine, coffee, tea, curries and tobacco. Extrinsic staining is the simple staining of the tooth's surface enamel or exterior and these are the easiest stains to remove. That last paragraph is very important and we would always recommend a discussion with your dentist first before under going any teeth whitening process. It is also important that you understand the process, and what your 'natural shade. actually was - you may as well have realistic expectations!

Our natural tooth colour is in the main determined by the shade of our dentin, and the thickness of our enamel. As we grow older and substances are absorbed through our enamel and into the (unfortunately) porous dentin our teeth appear darker as dentin darkens over time, and to make matters worse as we age our enamel gets thinner emphasizing the darker shade even more. Unlike the rest of our body the human tooth stops growing once they are fully formed and wear and tear does take place, and so it is vital that we look after all aspects of our 'Oral Hygiene' - the next step is false teeth!! Teeth Whitening Side Affects - Are they Harmful?


If you visit your dentist regularly for check ups and ask their advice you will find that teeth whitening is for most people. It is one of the most safest cosmetic procedures to whiten your smile. The side affects of teeth whitening are minimal in most cases, however in the main there are 3 areas that we should be concerned about;-

Tenderness of the Gums & Cheeks

With all of today's modern teeth whitening kits your gums and cheeks are protected by the whitening tray, mould or barrier material (strip). When the days treatment is complete the gums or cheeks can sometimes appear white or feel slightly sore or tender, HOWEVER DON'T PANIC. In most extreme cases the soreness should ease and disappear in the following 48 / 72 hours.

Sensitivity

There are some extreme cases of patients with unbearable pain from tooth sensitivity. If you know already that you have sensitive teeth then you should consult your dentist and take necessary precautions such as ;-

a) Use a pre whitening toothpaste and desensitising gel before, during and after the treatment

b) Use a 'sensitive' toothpaste after the treatment. Arm & Hammers 'sensitive' is a brilliant tooth paste for preventing tooth sensitivity.

In general sensitivity during or after tooth whitening can range from a mild tingling sensation to moderate discomfort. Generally we find that in these cases it can last from a few hours to two days. To help any sensitivity you can also try to avoid cold drinks and foods for this period.

Tooth sensitivity is not long lasting and is totally reversible, DO NOT PANIC - it does go away all by itself and at no stage is any damage done to the nerve or tooth. Whitening is a safe procedure when done correctly and the instructions followed carefully. There are minimal side affects and the benefits are endless.

White Spots

If you were born with 'white spots' (fluorosis stains) or had any pre existing white spots before whitening, these spots after a whitening treatment can in some circumstances appear 'bright white' against your gleaming white teeth. Generally these spots go back to their 'natural' shade after a few days. The reason people can have these spots is usually due to fluoride in the water, and they tend to turn alot whiter during the process due to the different moisture content from that of the tooth.

Veneer, Crowns, Caps & Fillings

For those of us that have any of these the good news is that the whitening process will not damage them - it will not whiten them however! just your natural teeth.

The bottom line is that teeth bleaching can cause some variations of sensitivity, however this is only a temporary experience.
In order to permanently bleach the teeth, the solution (carbamide or hydrogen peroxide) must penetrate through the enamel to reach the underlying dentin. The dentin contains tiny tubules which allow the flow of cellular fluids between the live nerve in the center of the tooth and the outer layer of enamel.  This is the reason that the teeth become sensitive.  It would be logical to assume that this process can't be too good for the health of the nerve, however, over the course of the years that these bleaching products have been used, no ill effects have been reported.  The sensitivity goes away within a few days of terminating the bleaching treatments, and the nerves in the teeth suffer no permanent damage.  The sensitivity, while temporary, in rare cases may be severe and has been known to force some patients to terminate the bleaching treatment early.

So finally in our analysis, what can't teeth bleaching do? 
  • Bleaching will not bleach out the black, brown or white color imparted to teeth due to decay.  Teeth should be repaired before bleaching is performed.

  • Bleaching will not bleach out darkness imparted to teeth by old amalgam fillings.  Removing the old metal filling and replacing it with a new composite will usually accomplish this, but if the tarnish has penetrated deeply into the tooth structure, the tooth may remain permanently discolored.

  • Bleaching will not generally improve the appearance of fluorosis if the patient grew up in a part of the country (before the 1960's) that had a high concentration of fluoride in the drinking water.  This problem is also prevalent in patients who "ate" a lot of fluoride toothpaste when they were toddlers.

  • Bleaching is ineffective in reducing the irregular gray horizontal lines seen on patients with tetracycline stain in their tooth structure.  Tetracycline stain is seen primarily in older patients who received tetracycline to treat ear infections when they were infants and toddlers.  Physicians in those days did not know that this drug would incorporate itself into the developing teeth of children causing this deformity.

Can I Whiten My Teeth?


In the main if you have maintained good healthy teeth and visited the dentist regularly it is more than likely that you will be suitable for teeth whitening. Indeed the vast majority of people are.

However there are a few conditions that may make teeth whitening inappropriate or could make teeth whitening sensitive;-

1) Active gum disease or receding gums

2) Under 16 years of age

3) If you have over brushed your teeth this can lead to the exposure of the dentine at the base of the tooth. Also excessive consumption of carbonated drinks.

4) If you are pregnant.

5) More severe forms of fluorosis (brown spots to dark patches) would require a different kind of treatment as they would not really benefit from normal teeth whitening, mild cases are not such a big deal.

The amazing fact of course still remains. If you placed a tooth in a can of fizzy drink it would dissolve completely over the course of 12 - 24 hours!

Dentists have been whitening teeth for hundreds of years using carbamide peroxide and hydrogen peroxide. Fundamentally it is safe apart from the aforementioned points, and we always recommend that you visit your dentist about your personal situation before under taking any whitening procedure.

The following guidelines  are for the use of professional quality thermo shrink whitening trays, dentist moulds and high quality dental strength bleaching gels.

Before Treatment
Ideally brush and floss prior to bleaching your teeth as the whitening process is more affective with clean, dry teeth. Make sure that the whitening trays are clean and dry and fit well before applying any whitening gel. Moisture can reduce the efficiency of the gel and can sometimes slow down the whitening process.

Applying the Whitening Gel
It is VERY IMPORTANT that you do not fill the whitening trays with gel. Simply place a small drop of the whitening ingredient half way up on the inner front surface of the tray where each tooth has left an impression. Do not spread the whitening ingredient around.
The average persons arch requires 0.5ml - 0.7ml of whitening gel, and we recommend that you only whiten the first 8-10 teeth as when we smile most people do not see the back teeth. However as everyone is different and our smiles differ it is a matter of choice how many you whiten. The whitening ingredient will mix and work its way around all the teeth during the process so a good coverage is always achieved.

Bleaching Trays
Insert the tray into the mouth and over the teeth. If  there is any excess whitening ingredient that seeps over the top of the tray, simply hold the tray in place with one hand and wipe away excess gel with a dry tissue from the lips and gums. Then wear the tray with the whitening gel as directed by the manufacturer.

After Whitening
Remove the tray and rinse away any excessive whitening gel. Then floss and brush with your ‘sensitive’ tooth paste or if the teeth are overly sensitive wear the trays for longer with the Dr Collins De-Sensitising gel.


Important Whitening Considerations

Dental Work.
Tooth bleaching or whitening will only whiten your teeth back to their natural shade, whatever that was. If you were born with naturally yellow teeth then that is the shade you will go back to. The whitening process works over a period of time and everybody is different, some see results or big shade changes in a matter of days, others who may have more intrinsic stains can take longer. The process generally can take from 5 - 30 days.

Tooth whitening will NOT work on any dental work, it will not whiten or bleach, caps, crowns, veneers, bridges,fillings, bonding or any other artificial dental work. It is recommended that once you under go the whitening process you then have your ‘dental work’ redone to match your new shade, knowing that you will always be able to maintain your new levels of whiteness with simply top up whitening.

Teeth that have become yellow over time generally whiten the fastest. However Tetracycline (antibiotic) stained teeth or greyish teeth are more difficult to whiten or bleach. If you have amalgam (silver) filling’s, near or behind the front teeth, long term bleaching can cause surrounding teeth to appear or have a green tinge to them. Also, exposed root portion of the teeth which occurs with gum recession also will not bleach. Bleaching teeth that are generally translucent in appearance can cause teeth to become more translucent or darker as the dentin (porous dark layer underneath the enamel) is more visible. if you are concerned about any of t he above it is recommended that you consult your dentist.

Sensitivity
As we have already mentioned some sensitivity during the whitening process is normal. Your gums can become slightly sensitive (tingling sensation) or can be moderately uncomfortable. This is common and while using all products containing peroxide based gels, however it is NOT harmful. If you suffer from increased sensitivity then follow our instructions on how to prevent sensitivity. Due to the nature of the whitening process crestwhitestrips.co.uk highly recommend the use of Dr Collins desensitising gel following a bleaching treatment.
